Employee refuses to sign job description


An employee is given a baseline audiogram as part of an employment medical examination. He/she works in the plant for ten years with documented noise exposures below 85 dBA TWA. Each year, the employee is offered but refuses an audiogram as part of a proactive, non-required hearing conservation program. Ask the employee to write a statement recording their refusal to sign the document or, if they refuse to do that too, ask a company representative to state that the employee has refused to sign the acknowledgement form and to sign and date this statement.
